A free telephonic service
offered to assist Members searching for quality outcomes and cost savings.
This exclusive program allows Members to shop for care based upon doctor
and facility procedure outcomes and analyze costs for procedures which can
vary widely by facility. This benefit is great as it allows the member to
call Avalon to inquire about a medical facility's cost and obtain specific
details on a doctor such as how many mal practice suites that doctor has
had, the doctors success vs. fail rate. Avalon does all the research and
provides the member with this critical information so the member can make
the best decision.

two days up to two months.

Preventive Care benefit is
payable after a six (6) month waiting period and 15 months waiting period for maternity.
