in Miami-Dade, Broward, Palm Beach, Martin and St. Lucie counties and 21

Maternity Coverage
If you live in South Florida in the following
counties Miami-Dade, Broward, Palm Beach, Martin, and St. Lucie you can apply
for the best maternity package available in Florida. This maternity is
optional and must be added to the base plan. When obtaining a quote in your
area once you find a plan that fits your needs and budget the maternity benefit
can be added to the base price for only $110.00. The additional cost to have
maternity is only $110.00.
Benefits
Maternity coverage includes obstetrical and maternity care received on an
inpatient or outpatient basis including medically necessary prenatal and
postnatal
care of the mother. This benefit also includes post delivery care including a
postpartum assessment of the mother and the performance of any medically
necessary clinical tests and immunizations in keeping with prevailing medical
standards and may be provided at the hospital, at the attending physician’s
office, at an outpatient maternity center, or in the home by a qualified
licensed health care professional trained in mother and baby care. Coverage
includes
the services provided in a licensed birthing center and the services of
certified nurse-midwives and licensed midwives.

Copayment. All visits and test after are covered 100%
Maternity Post Natal Care: No Copayment
Hospital Facility/Birth Center
Hospital services include, but are not limited
to anesthesia, x-rays and laboratory services for conditions
relating to the pregnancy:
Normal/Cesarean Section Delivery $1,000
Copayment. This means to deliver a baby the cost to you is
$1,000.00.
Waiting Period
Maternity Coverage is effective fifteen (15)
months after the individual’s effective date of coverage under
this Rider. This means you must not be pregnant
at the time of application and you must have the insurance for
fifteen (15) months. After applying for the maternity coverage
you must wait fifteen (15)
months before Vista covers any charges related to pregnancy. If
you are currently pregnant you will not be able to
apply for this coverage. The insurance company will not pay any claims relating to pregnancy if the plan is not in force for
fifteen (15) months.

Apart from the monthly cost. The actual cost
to the delivery of the baby is $1,000.00. Regardless of plan selected.
