  • Unlimited office visits with a $40 co-pay per member at participating providers with Annual Deductible Waived

  • $10 Generic Drugs (No Deductible)

  • Emergencies (In Network)  ER Copay $150 for each visit

  • Extensive PPO network

  • Medical, Dental, and Vision Included

  • Lifetime maximum of up to $5 million

the largest health benefits company in the nation. Sound members have access to a large PPO network that includes over

25,000 doctors and more than 500 hospitals. Sound members also get access to lots of other services to help them stay

healthy, and there’s even a special Customer Service number just for them.

 

For 19-29 year olds, rates can be just $98-$190 per month, depending on the plan you pick, where you live, your age, medical history and gender.* That’s not much to pay for some peace of mind.

  • Unlimited office visits with a $30 co-pay per member at participating providers with Annual Deductible Waived-
    does not include lab and x-rays
  • Lifetime maximum of up to $5 million
  • Out-of-Pocket Maximums provide protection against catastrophic expenses
  • $10 Generic Drugs (No Deductible)
  • 100% coverage up to $300 with Annual Deductible Waived* for Preventive Care for children (through age 6) and Pap smear, mammograms and PSA for adults.
    *After maximum benefit is met, deductible and coinsurance will apply,
    except Well Baby Immunizations & Lab (through age 6) which is NOT subject to $300 Preventive Care Maximum (State mandate).

  • Based on your medical conditions the insurance companies may exclude treatment for certain health conditions, and or charge an additional premium.

  • The approval time can vary from two days up to two months

  • The Application Fee $25 is waived when you apply online

  • A $5.00 administrative fee will be assessed to generate a paper bill. Choose Monthly Automatic Bank Withdrawal under payment type to avoid this fee when applying.

  • Pre-Existing conditions - Coverage will not be provided for the 12 months following the effective date of the plan for medical conditions that existed in the 12 months prior to the effective date. UniCare will, however, give you credit for the time an insured person was covered by any other creditable coverage that was in effect during the 18 months preceding either UniCare's Effective Date of Coverage or the date UniCare received a substantially completed application, if the most recent creditable coverage was under a group health plan, governmental plan, or church plan.
